In the light of WHO declaring the COVID-19 virus to be a pandemic, the INDIAN DENTAL ASSOCIATION (IDA) recommends preventative measures for dental professionals to minimize transmission through contact and dental procedures --- as scientific information leads to improvements in infection control, risk assessment, and disease management.

Dental chambers are the most prone sites that a patient might get COVID-19 as an unwanted souvenir. 

Presumably India is now going through the most affected stage of Corona virus spread. Unknown carriers and asymptomatic patients are the mostly causing the spread unaccounted. No one is sure enough about their status regarding COVID-19. 

Dentists are inescapably exposed to patients' saliva, sputum and blood as there work is concerned within the oral cavity. There are no chances to escape from the infection if an unknown or asymptomatic patient arrives at the clinic. The most possible transmission routes of COVID-19 are Direct or Indirect transmission.

The risk of direct transmission involves face to face communication and frequent exposure to saliva, blood and other body fluids. This is the most effective way of transmission from one patient to the doctor and from the doctor to other patients. 

If we project this spread graphically it shows, 

It's a probability chart of infection spread from a dentist who is used to meet 10 patients a day. If the dentist become an asymptomatic carrier then the spread will show a spike rise. In case if the dentist is symptomatic then the spread will be less as the dentist will go through the isolation protocols. 

 

Indirect spread or contact spread is another mode of COVID-19 transmission that may occur due to the saliva or aerosols present in the air inside the clinic or on the surfaces. Traces of saliva or fomites can cause such damage. Hence strict clinical guidelines are implemented on the practicing dentists.

Classified treatments

Right now DCI has classified dental treatments as Emergency Dental Treatments, and Non-Emergency treatments. 

What are the Emergency dental treatments?

1) Excessive or uncontrolled bleeding from mouth due to trauma or severe infection.

2) Ludwig's angina or Cellulitis (Excessive swelling of face due to infection)

3) Tooth fracture due to trauma.

4) Acute exacerbation of chronic pulpitis. (Severe toothache) 

5) Pericoronitis or pain related to wisdom teeth

6) Dry socket or alveolar osteitis

7) Fractured old restoration causing severe pain and swelling.

Emergency dental treatments are also classified into 2, Aerosol generating procedures and Non-aerosol generating procedures.

** Aerosol Generating Procedures or AGP are like Filling or restoration procedures, Root canal treatments, Surgical extractions, Scaling etc.

** Non-aerosol Generating Procedures are like consultations, fabrication of removable dentures, simple extractions etc.

As the DCI guidelines imply every patient should go through these following steps.

1) Classify the patients as emergency or non-emergency dental care cases.

2) Primary care should follow a triage like Advice > Analgesics (Pain killers) > Antimicrobials/ antibiotics.

3) If the discomfort doesn't resolve within 72 hours then call the patient for an elective treatment.

করোনা আতঙ্কের আবহে সারা বিশ্বব্যাপী এখন মানুষের দৈনিক চাহিদা তালিকা অনেকাংশে পাল্টে গেছে।তাঁরা এখন নিয়মিত ফুচকার দোকানে দাঁড়িয়ে ফুচকা খেতে ভয় পায়। এমন কি অটো বা বাসে উঠলে সহযাত্রীকেও ভয় পায়। এই করোনা ভাইরাসের দৌলতে মাসকাবারি মালের সঙ্গে বা নিত্য প্রয়োজনীয় ওষুধের সাথে এই বিশেষ জিনিসটি এখন সবাই কেনে। দরকারে ঘর ভর্তি করে রাখার গুরুত্ব অনুভব করে। আজ থেকে প্রায় ৭০ বছর আগের আবিষ্কৃত একটি জিনিস, যা সকলের নজর কেড়েছিল ২০০০ শালে এসে। আর নিত্য ব্যাবহারের স্বীকৃতি পেল ২০০২ শালে এসে। খুব কম চাহিদার তালিকা ভুক্ত একটি গুরুত্বপূর্ণ জিনিস যেন রাতারাতি প্রাধান্য শিখরে পৌঁছে গেল। আর আজ দাঁড়িয়ে তার বিকল্প খোঁজার কোন উপায় নেই কোথাও। হ্যাঁ মশাই জিনিসটি আমাদের করোনা আবহের বহুল ব্যাবহৃত Hand Sanitizer। 

একটি পরিসংখ্যান বলছে, করোনা পূর্ব সময়ে sanitizer এর চাহিদা ছিল ৬০-৭০%। মূলত হাসপাতাল ও বিভিন্ন ডাক্তারখানায় এর ব্যাবহার সীমিত ছিল। কিন্তু প্রথম করোনা রুগী সংক্রমণ শুরুর পরে এই চাহিদা  জানুয়ারি মাসে  বৃদ্ধি হয়ে দাঁড়ায় প্রায় ৩০০%। আর মার্চ পরবর্তী সময় থেকে?? গত বছরের মার্চ এর তুলনায় sanitizerএর চাহিদা এখন ৪৭০% বেশী। আর ইতিমধ্যে শুধুমাত্র আমেরিকা দেশে স্যানিটাইজার কোম্পানি প্রায় 200 million ডলার মুনাফা করে বসেছে। পরিসংখ্যান বলছে এই ব্যাবসা থেকে এই বছর অধিকাংশ বড় কোম্পানি প্রায় ২ billion ডলার মুনাফা করতে পারে। তবেই বুঝুন কাণ্ড। 

Sanitizer বৃত্তান্ত

জল ও সাবান দিয়ে হাত পরিষ্কার করার কোন বিকল্প নেই। কিন্তু সব জায়গায় তো আর হাত ধুয়ে নেওয়া সম্ভব নয়। এই চিন্তা ভাবনা থেকেই ১৮০০ শতকে প্রথম অ্যালকোহল দিয়ে হাত ধুয়ে কাজ করার পরিকল্পনা করা শুরু হয়। 

আধুনিক ডাক্তারি কাজে উপযুক্ত Hand sanitizer প্রথম নিজ রূপ পায় ১৯৬৬ শালে। ক্যালিফোর্নিয়া শহরের বেকার্সফিল্ড এ Lupe Hernandez প্রথম hand sanitizer এর পরিকল্পনা করেন এবং অ্যালকোহল এর সাথে জেল মিশিয়ে ডাক্তারদের জন্যে এই স্যানিটাইজার বানিয়ে দেন। মতান্তরে অনেকে বলেন ১৯৬৬ নয় ১৯৬০ এ Hernandez প্রথম হ্যান্ড স্যানিটাইজার বানিয়েছিলেন। 

এর পরেই আসে একটি বিখ্যাত নাম, Sterillium।

 

 ইতিহাস ও সেই কথাই বলে। জার্মানির Hartman কোম্পানির ঘর থেকে তৈরি হয় বিশ্বের প্রথম উপযুক্ত এবং নিখুঁত অ্যালকোহল বেস হ্যান্ড স্যানিটাইজার স্টেরিলিয়াম। সময়টা ১৯৬৫, ৭৫% অ্যালকোহল এবং glycerin সহ এই hand sanitizer সারা ইউরোপে খ্যাতি পায়। আজকের দিনেও Sterilliumএর ভূমিকার সাথে কারো তুলনা চলেনা। আমাদের বাঙ্গালীর নবাব গেঞ্জির মতই একেও চোখ বন্ধ করে ভরসা করা যায়। 

প্রকৃত Hand sanitizer আবিষ্কারক কে, সেই বিষয়ে Joyace Bediঅনেক অনুসন্ধান করেছিলেন। সেই অনুসন্ধানে আরেকটি গুরুত্বপূর্ণ ঘটনা উঠে আসে। 

রঙের মিস্ত্রী বা গাড়ি কারখানার মিস্ত্রীদের লক্ষ্য করলে দেখবেন। কাজের শেষে তাঁরা তারপিন তেল বা কেরোসিন বা বেঞ্জিন দিয়ে নিজেদের হাত পরিষ্কার করে নেয়। ১৯৪৬ এর মাঝামাঝি আমেরিকার এক দম্পতি Goldie Lippman and Jerry Lippman রাবার ফ্যাক্টরি তে কাজ করতে করতে নিজেদের ব্যাবহারের জন্যে একটি জেল বা স্যানিটাইজার বানিয়েছিলেন। এর মূল উপাদানগুলির মধ্যে ছিল, পেট্রোলিয়াম জেলি, মিনারেল অয়েল, আর ৫% অ্যালকোহল। এই একই সংস্করণ আজও গাড়ির কারখানা বা রঙের কারখানায় ব্যাবহার হয়। প্রথম দিকে ছোট ছোট জ্যাম বা জেলির শিশিতে ভর্তি করে এই দম্পতি তাঁদের বানানো স্যানিটাইজার বিক্রি করতেন। ক্রমে এর প্রাধান্য বাড়তে থাকায় এই স্যানিটাইজার এর নাম দিলেন "gojo"। স্টেরিলিয়ামের গুরুত্ব সারা বিশ্বব্যাপী ছড়িয়ে পরায় এই দম্পতি তাঁদের কোম্পানির ঘরে একটি নতুন পণ্যের জন্ম দিলেন। ৭০% ইথাইল অ্যালকোহল আর প্রপিলিন গ্লাইকল এর মিশ্রণে বানানো এই স্যানিটাইজারের নাম দেওয়া হল Purell। যা আজও সারা বিশ্বের সবচেয়ে বেশী বিক্রি হওয়া স্যানিটাইজার গুলির মধ্যে অন্যতম।

প্রতিদ্বন্দ্বী প্রতি মুহূর্তে আমাদের পাশে চলে আসে লড়াই এর ময়দানে। Purellএর সাফল্য দেখে Vi-Jon Industry তাঁদের কোম্পানি থেকে GermXবলে একটি স্যানিটাইজার নিয়ে আসে। সারা বিশ্বব্যাপী বিক্রির খতিয়ান দেখলে দেখা যাবে ভি জন ইন্ডাস্ট্রির হ্যান্ড স্যানিটাইজার দ্বিতীয় স্থানে আছে, Purellএর পরেই।

প্রথম থেকেই কি এমন ছিল?

কোথায় মশাই? আগে তো হ্যান্ড স্যানিটাইজার নিয়ে এত রমরমা ব্যাবসার কথা শুনিনি?

আরে সাধারণ consumer এর কাছে যে এই প্রোডাক্ট নিয়ে আসার সিদ্ধান্ত হল ১৯৯০ শালে। তারপরেও পেরিয়ে গেছে দশটি বছর। একটি সাধারণ ওষুধের দোকানের পণ্য হিসেবেই সীমাবদ্ধ ছিল স্যানিটাইজার। কোম্পানি প্রথম লাভের অঙ্ক দেখতে পেল ২০০০ শালে এসে। তারপর ২০০২ শালে CDC নিজেদের guidelines হ্যান্ড স্যানিটাইজার ব্যাবহারের উপর প্রাধান্য দিল। ব্যাস আর যায় কোথায়? চাহিদার সাথে উৎপাদন বৃদ্ধি পেল। যদিও সাধারণ মানুষ সে অর্থে ওয়াকিবহাল ছিলেন না। বিশেষ করে ভারতীয় মানুষ তো নয়ই। ২০০৯ শালে WHOযখন তাঁদের নির্দেশিকায় স্যানিটাইজারের ভূমিকা নিয়ে ভূয়সী প্রশংসা করে শংসাপত্র দিল। স্যানিটাইজারের বাজার বৃদ্ধি পেল। হাসপাতালের চৌহুদ্দি ছেড়ে স্যানিটাইজার তখন মধ্যবিত্তের গেরস্থালী বা কাজের জায়গায় ঠাই পেল।  

চাহিদা তবুও ক্ষুদ্রতর বাজারে ছিল। সারাবিশ্বে সাধারণ মানুষের বিচারে মাত্র ১ লক্ষ ৫২ হাজার গ্রাহক ছিল বিভিন্ন কোম্পানির হ্যান্ড স্যানিটাইজারের। H1N1 ভাইরাসের প্রকোপ বৃদ্ধি মাত্র এই সংখ্যা ৫ লক্ষ ৭৬ হাজার পেরিয়ে যায়। আর বর্তমান বিশ্বে হ্যান্ড স্যানিটাইজারের চাহিদা? আমরা প্রত্যেকেই এই ঘটনার সাক্ষী।

কি ভাবে কাজ করে স্যানিটাইজার?

সামগ্রিক চিত্রপটে না গিয়ে করোনা ভাইরাস প্রসঙ্গেই বলি। করোনা ভাইরাসের গঠনগত মূল তিনটে উপাদান হল একটি RNA, প্রোটিন ও লিপিড এর কাঠামো। 

সংক্রামিত রুগী যখন কাশেন বা হাঁচি দেন, বা কথা বলেন, এই ভাইরাস তার শ্বাস প্রশ্বাসের মাধ্যমে বাইরে এসে বাতাসে ছড়িয়ে পরে। সব থেকে খুদ্র বাহক কণার সাহায্যে এই ভাইরাস প্রায় ১০ মিটার অব্দি ছড়িয়ে পরতে পারে। অপেক্ষাক্রিত ভারি বাহককণা কিছুক্ষণ বাতাসে স্থায়ী হয়ে নিচে পরে যায়। সেটা মেঝে বা টেবিল, চেয়ার, বাসের হাতল ইত্যাদি যা কিছু হতে পারে। এমনকি টাকা পয়সা, খাম, কাগজ, বাক্স ইত্যাদিও। একজন সুস্থ ব্যাক্তি যদি এমন কোথাও হাত দিয়ে ফেলেন তখন তার হাতের চামড়ার সাথে এই ভাইরাস আটকে যায়। চামড়ার মতন নিশ্চিন্ত বাসস্থান কোথাও নেই। মৃত চামড়ার কোষ বা ঘাম ইত্যাদির সাহায্যে ভাইরাস চামড়ার উপরে একটি চিটচিটে আঠালো পদার্থের সাহায্যে আটকে যায়। 

হিসেব বলে, সারাদিনে একজন মানুষ প্রতি মিনিটে অন্তত দুবার নিজের অজান্তেই মুখে, চোখে বা নাকে হাত দিয়ে ফেলেন। করোনা ভাইরাস এই সুবর্ণ সুযোগের অপেক্ষাতেই থাকে। একবার মানুষের শ্বাস যন্ত্রে প্রবেশ করতে পারলেই ভাইরাস নিজের স্বীয় ভূমিকায় কর্মঠ হয়ে ওঠে। আর ব্যাস, এই ভাবেই শুরু হয়ে যায় সঙ্ক্রমণের ক্রমবৃদ্ধি। 

শুধু মাত্র জল দিয়ে হাত ধুয়ে নিলে এই ভাইরাস যায়না। তার প্রধান কারণ আমাদের চামড়ায় প্রস্তুত সেই আঠালো মাধ্যম যার সাহায্যে ভাইরাস আটকে থাকে। সাবান এই জায়গায় একটা গুরুত্বপূর্ণ ভূমিকা নিয়ে কাজ করে। সাবানের মধ্যে ফ্যাট জাতীয় উপাদান থাকে। এদের amphiphile বলা হয়। এই ফ্যাট জাতীয় বস্তু করোনা ভাইরাসের লিপিড অংশের সাথে দ্বন্দ্ব শুরু করে, এবং যথারীতি ভাইরাসের লিপিড অংশ ভেঙ্গে যায় ও গোটা ভাইরাসের ক্লাস্টার বা সমষ্টি তাসের ঘরের মতই ভেঙ্গে পরে। 

সঠিক মাত্রার অ্যালকোহল যুক্ত হ্যান্ড স্যানিটাইজার ঠিক এই কাজ করেই করোনা ভাইরাস থেকে আমাদের বাঁচিয়ে দেয়। আর ঠিক এই কারণেই হ্যান্ড স্যানিটাইজারের ব্যাবহার এখন এই মহামারীর মাঝে আবশ্যিক। 

সঠিক স্যানিটাইজার চিনব কি করে??

আগেকার দিনে দুরকম হ্যান্ড স্যানিটাইজার ব্যাবহার হত, একটি জৈব বা আয়ুর্বেদিক, অপরটি কৃত্রিম। organic আর chemical। অরগানিক এর মধ্যে টক কমলা লেবু, ল্যাভেন্ডার, Thyme ইত্যাদি ব্যাবহার করা হত। বিয়ে বাড়িতে আজও কিছু জায়গায় পাতের শেষে গরম জলে পাতি লেবুর টুকরো দেওয়া হয়। তেল চিটচিটে হাত নিমেষে যেন বেশ হাল্কা বলে মনে হয়। বিয়ে বাড়ি স্পেশাল স্যানিটাইজার বলতে পারেন।

বর্তমানে বাজারে দুরকম স্যানিটাইজার পাওয়া যায়। একটি হল Alcohol based, আরেকটি Non-alcohol based। 

Non-alcohol based স্যানিটাইজার অনেক ক্ষেত্রে organic দাবী নিয়ে বাজার মাতিয়ে দেয়, কিন্তু তার কার্যকারিতা সম্পর্কে নিঃসন্দেহ থাকা মুশকিল। তাতে যে কি আছে আর কি নেই সেই বিশাল অজানা সমুদ্রে মন্থন করে খুঁজতে হয় ওতে আদৌ অ্যালকোহল আছে কি না। নিম পাতা, তুলসী পাতা, লবঙ্গ, পুদিনা, ধনে, সর্ষে, মৌরি, aloevera, গোলাপ জল, বৃষ্টির জল, সাত সমুদ্রের জল ইত্যাদি কত। হীরক রাজার সেই ডায়লগ মনে পরে যায়, "কি নাই সে ঘরে? চুহা, সাপ ব্যাঙ, শকুনির ঠ্যাং।" (মজা করছিলাম। কিছু মনে করবেন না) 

আসল যুক্তিযুক্ত থিওরি মত অনুযায়ী এবং বিভিন্ন গবেষণার দ্বারা প্রমাণিত তথ্য অনুযায়ী অ্যালকোহল বাদ দিয়ে কোন স্যানিটাইজার অ্যালকোহল যুক্ত স্যানিটাইজারের বিকল্প হতে পারেনা। হাত পরিষ্কারের জন্যে সেগুলি উপযুক্ত হতেই পারে। বিভিন্ন ফুল ও ফলের নির্যাসের কারণে সুগন্ধি হিসেবেও ব্যাবহার হতে পারে। কিন্তু করোনা?? কোন এমন তথ্য এখনও অব্দি সিদ্ধ নেই। 

অ্যালকোহল কত থাকতে হবে?

একটি স্যানিটাইজারে নিদেন পক্ষে হলেও ৬০-৮০% অব্দি অ্যালকোহল থাকতেই হবে। 

কি কি অ্যালকোহল ব্যাবহার হয়? 

মূলত ইথানল বা ইথাইল অ্যালকোহল। এছাড়া ব্যাবহার হয় প্রপানল, বা আইস প্রপাইল অ্যালকোহল। এদের সাথে আনুসাঙ্গিক উপকরণ থাকে। গ্লিসারিন হতে পারে, Chlorhexidine হতে পারে, সুগন্ধি কিছু হতে পারে ইত্যাদি। কিন্তু মূল উপাদান অ্যালকোহল ৬০-৮০%। ৭০% অ্যালকোহল যুক্ত স্যানিটাইজার করোনা ভাইরাসের জন্যে উপযুক্ত। তার নিচে খুব একটা কার্যকরী নয়। 

কিছু বিভ্রান্তি।

বিভিন্ন কোম্পানি নিজদের জিনিস বিক্রির স্বার্থে লেবেলের গায়ে অ্যালকোহলের মাত্রা সঠিক ভাবে লেখে না। অনেকে তো শুধু নাম উল্লেখ করে কিন্তু আর কিছুই লেখা থাকে না। কিছু কিছু কোম্পানি একটি বিভ্রান্তি করে এই ভাবে লিখে, w/w, w/v, v/v.

w/w: Weight by weight or weight concentration. কোন মিশ্রণ এর কোন উপাদান কি পরিমাণে রয়েছে তার হিসেব জানানোর জন্যে এই সূচক ব্যাবহার করা হয়। ১০০ গ্রাম এর চাল ও ডালের মিশ্রণে যদি ৩০ গ্রাম ডাল থাকে, তাহলে সেই মিশ্রণে ডালের মাত্রা লেখা হবে ৩০% w/w এই হিসেবে।

w/v: weight per volume. একে অনেক সময় mass per volume ও বলা হয়। ১০০ মিলি লিটার জলে যদি ১ গ্রাম চিনি মেশানো হয়ে থাকে তাহলে ওই মিশ্রণে চিনির উপস্থিতির সূচক, ১% w/v

v/v: volume per volume, একটি মিশ্রনের দুটো উপাদান তরল হলে এই সূচক ব্যাবহার করা হয়। ৫০ মিলি লিটার ইথাইল অ্যালকোহলের সাথে যদি ৫০ মিলি লিটার গ্লাইকল মিশিয়ে একটি মিশ্রণ তৈরি করা হয় তাহলে ওই ইথাইল অ্যালকোহল এর সূচক হবে ৫০% v/v। 

কোন রকম তথ্যের ভুল নজর কারলেই প্রশ্ন করুন। প্রয়োজনে ফিরে আসুন। টাকা নষ্ট করে দরকার নেই। 

কিছু স্যানিটাইজার যা ভারতীয় বাজারে এখন বহুল ব্যাবহৃত। 

Dettol alcohol based Hand sanitizer

 

 মূল উপাদান গুলি একবার দেখা যাক,

Ingredients: Denatured Alcohol- 69.4% w/w,
Water PEG/PPG-17/6 copolymer,
Propylene glycol,
Acrylate /C10-30 alkyl acrylate, cross polymer,
Tetrahydroxpropyl ethylenediamine, Perfume.

Denatured alcohol আসলে ইথাইল অ্যালকোহল কে পানের অযোগ্য করার উদ্দেশ্যে বানানো হয়েছিল। ১০% মিথাইল অ্যালকোহল মিশিয়ে দিলে ইথাইল অ্যালকোহল বিষাক্ত হয়ে যায়। একেই Denatured alcohol বলছে। আর তার পরিমাণের হিসেব লেখাই আছে।

 Ingredients: Ethyl alcohol 95% v/v or 62% w/w (তফাৎ টা খেয়াল করতে হবে)

Isopropyl alcohol 3% w/w,

Dexamethasone, a drug known to many of us, is basically known as Decadron. On June 16, 2020, a report from Oxford University in England found that the use of the drug Dexamethasone had significantly reduced the mortality rate of critical patients.

After administering the drug to about 2,000 patients, it was found that the mortality rate of critical patients on the ventilator could be reduced from 40% to about 28%. And it has been possible to reduce the mortality rate of patients who were completely dependent on oxygen support for about 20%.

What is Dexamethasone??

Dexamethasone is a type of corticosteroid drug that has been shown to benefit multiple diseases. Significant application of this drug is seen in the subsequent treatment of Allergy, Arthritis, various skin diseases, eye problems, stomach problems, major operations etc.

In another case dexamethasone has played an important role in treatments of respiratory problems. And right from the start of its application, dexamethasone has successfully become a life-saving drug for critically ill COVID-19 patients.

No role of this drug has been observed in infected patients except in critical patients.

How does dexamethasone work?

COVID-19 disease actually causes inflammation of our airways and lungs. Once the inflammation becomes more severe, a condition called cytokine storm occurs in the body.

At very small doses dexamethasone can prevent this cytokine storm and become life-saving drug for the patient.

What is cytokine storm ??

The term was first mentioned by J L Ferrera in a 1993 magazine. The subject of his research was graft vs. Host disease.

Our body develops a normal immunity to any kind of disease. Cytokine is a special chemical that is identified as a signaling factor in this immune system.

The presence of this cytokine in the body is at a very normal level. Cytokine is made to prevent any disease so that the body is always ready. If the level of this cytokine in the body is increased in any way, various organs of the body lose their ability to function and gradually a patient dies.

Relevance?

It has been found that the human body's immune system begins to weaken with age. This is called immune aging or immune senescence. Over time, interacting with various elements of the environment over a long period of time, the body is found to be in a chronic inflammatory state. This condition is called inflammaging.

In most older patients, signaling factor cytokine is present in much higher levels than normal. The body can adapt to this condition. But in some physiological diseases, especially inflammation of the airways caused by infection with viruses, the levels of this cytokine begin to rise. Due to age, this cytokine remains unused most of the time because the immune system loses some of its ability to function. The presence of this excess amount of cytokine in the blood is called cytokine storm.

The importance of dexamethasone?

Infection with the H5N1 influenza virus, or SARS CoV-1, has been shown to cause cytokine storms. From that source, the incidence of cytokine storm was found during the examination of COVID-19 patients.

Dexamethasone can easily counteract this cytokine storm. In this way dexamethasone reduces the level of cytokine in the patient's body and saves the patient from multi-organ failure, and the patient can be brought back from the grip of death.

Is this system effective for all patients?

Absolutely not. According to the information available so far, it has been used in only 1/3 of the patients. Dexamethasone cannot be applied to any random patients.

Any patient may have an allergy to this drug. Dexamethasone can make other physical problems worse. Many times a patient may react to dexamethasone with all the medications he takes regularly and push the patient towards death. So its use has to be done very carefully. This medicine should not be used without the advice of a doctor.

One more thing to note is that, according to Oxford experts, this drug is only effective for critical patients of  COVID-19. It is not for normal healthy people or for patients with no symptoms.

Decamethasone is a very expensive drug ??

Absolutely not. This drug is widely used all over the world and its cost is very low. In the case of one patient, the limit of application of this drug may be about 10-15 days.

Applying dexamethasone in the same amount for 10 days costs only ₹ 480 to ₹ 600 per patient. These costs may increase or decrease depending on the location and the need for ancillary components. Dexamethasone alone is not effective to control COVID-19.

What are the side effects of this medicine?

The list of side effects of dexamethasone is quite long. But at the most urgent moment, it is applied only after considering its side effects as a life-saving medicine. However, those side effects seem insignificant in the face of life and death. But if a healthy normal person starts applying it to his body unnecessarily and unknowingly, then more than one bad symptom can start, starting from nervous weakness.

This news is quite comforting in this crisis of corona infection.

Complete research on the application of this drug and its results has not yet been obtained. This discovery is truly exciting news in the absence of a comprehensive idea of ​​any vaccine or proper treatment for COVID-19. Hopefully soon we will be able to overcome this crisis and get used to normal life again.

Dexamethasone, আমাদের বহু পরিচিত একটি ওষুধ যা মূলত Decadron নামে আমরা প্রায় সবাই জানি। ১৬ জুন ২০২০, ইংল্যান্ড এর Oxford University থেকে প্রকাশিত একটি পত্রিকায় জানা গেল এই Dexamethasone ওষুধটি প্রয়োগের পরে critical patient এর মৃত্যুর হার অনেকটা কমিয়ে আনা সম্ভব হয়েছে। 

প্রায় ২০০০ রুগীর উপর এই ওষুধের প্রয়োগ করার পরে দেখা গেছে যে, ventilator এ থাকা critical রুগীর মৃত্যুর হার যেখানে ৪০% ছিল তা প্রায় ২৮% অব্দি কমিয়ে আনা সম্ভব হয়েছে। আর যে সকল রুগী সম্পূর্ণ রূপে oxygen নির্ভর চিকিৎসাধীন ছিলেন তাদের মৃত্যুর হার প্রায় ২০% অব্দি কমিয়ে আনা সম্ভব হয়েছে।

Critical রুগী ব্যতীত শুধুমাত্র সংক্রামিত রুগীর জন্যে এই ওষুধের কোনো ভূমিকা দেখা যায়নি।

কিভাবে কাজ করে dexamethasone?

COVID-19 রোগটি আসলে আমাদের শ্বাসনালী ও ফুসফুসের প্রদাহ বা inflammation করে। এক সময় এই প্রদাহ অনেকটা গুরুতর রূপ নিলে শরীরে cytokine storm বলে একটি অবস্থার সৃষ্টি হয়। 

খুব সামান্য dose এ dexamethasone এই cytokine storm কে আটকাতে পারে এবং রুগীর জন্যে জীবনদায়ী হয়ে ওঠে।

Cytokine storm কি ??

১৯৯৩ শালের একটি পত্রিকায় J L Ferrera প্রথম এই শব্দটির উল্লেখ করেন। তাঁর গবেষণার বিষয় বস্তু ছিল graft vs. Host disease। 

যে কোন রকম রোগের জন্যে আমাদের শরীরে একটি স্বাভাবিক নিজস্ব প্রতিরোধ ক্ষমতা গড়ে ওঠে। Cytokine এক ধরনের বিশেষ chemical যাকে এই প্রতিরোধ ক্ষমতার signalling factor হিসেবে চিহ্নিত করা হয়।

শরীরে এই cytokine এর উপস্থিতি খুব স্বাভাবিক মাত্রায় থাকে। যে কোন রকম রোগ প্রতিরোধে যাতে শরীর সব সময় প্রস্তুত থাকে সেই জন্যেই cytokine তৈরি হয়। কোন রকম ভাবে শরীরে এই cytokine এর মাত্রা অতিরিক্ত বেড়ে গেলে শরীরের বিভিন্ন অঙ্গ কার্য ক্ষমতা হারিয়ে ফেলে এবং ক্রমে একজন রুগীর মৃত্যু হয়। 

প্রাসঙ্গিকতা?

দেখা গেছে, বয়স বৃদ্ধির সাথে সাথে মানুষের  শরীরে প্রতিরোধ ক্ষমতা বা immune system দুর্বল হতে শুরু করে। একে immune aging বা immune senescence বলে। সময়ের সাথে সাথে দীর্ঘকাল ধরে পরিবেশের নানান উপাদানের সাথে interaction করতে করতে বয়স্কালে এসে দেখা যায় শরীরে একটি chronic inflammatory state এ রয়েছে। এই অবস্থান কে inflammaging বলা হয়।

অধিকাংশ ক্ষেত্রে এই অবস্থানে দেখা গেছে রুগীর শরীরে এই cytokine জাতীয় প্রতিরোধ ক্ষমতার signalling factors স্বাভাবিকের তুলনায় অনেক বেশি মাত্রায় উপস্থিত রয়েছে। শরীর এই অবস্থাকেও নিজের মতন করে মানিয়ে নিয়ে চলতে পারে। কিন্তু কিছু কিছু শারীরিক রোগ, বিশেষ করে ভাইরাসের দ্বারা সংক্রমনের কারণে হওয়া শ্বাসনালীর প্রদাহে এই cytokine এর মাত্রা ক্রমে বাড়তে শুরু করে। বয়েসের কারণেই এই cytokine অব্যবহৃত থেকে যায় অধিকাংশ সময় কারণ immune system কিছুটা হলেও কার্য ক্ষমতা হারিয়ে দুর্বল। রক্তের মধ্যে এই অতিরিক্ত পরিমাণে cytokine এর উপস্থিতি কে cytokine storm বলা হয়।

Dexamethasone এর গুরুত্ব?

H5 N 1 influenza virus, বা SARS CoV-1 এর সংক্রমন হলে cytokine storm লক্ষ্য করা গেছে। সেই সূত্র ধরেই COVID-19 রুগীর ওপর পরীক্ষার সময় cytokine storm এর ঘটনা দেখতে পাওয়া যায়। 

Dexamethasone খুব স্বচ্ছন্দে এই cytokine storm কে প্রতিহত করতে পারে। এই ভাবেই রুগীর শরীরে cytokine এর মাত্রা কমিয়ে রুগীকে multi organ failure এর হাত থেকে বাঁচায় dexamethasone, আর রুগীকে মৃত্যুর কবল থেকে ফিরিয়ে আনা যায়। 

সব রুগীর জন্যেই এই ব্যবস্থা কার্যকরী?

একেবারেই না। এখনো অব্দি পাওয়া তথ্য অনুযায়ী মাত্র ১/৩ রুগীর ক্ষেত্রে এর ব্যবহার সম্ভব হয়েছে। যে কোন রুগীর শরীরে dexamethasone প্রয়োগ করা যায়না।

কোন রুগীর এই ওষুধে allergy থাকতে পারে। কারো আবার অন্যান্য শারীরিক সমস্যাকে dexamethasone আরও খারাপ দিকে ঠেলে দিতে পারে। অনেক সময় একজন রুগী নিয়মিত যে সমস্ত ওষুধ খান তাদের সাথে dexamethasone প্রতিক্রিয়া করে রুগীকে মৃত্যুর দিকে ঠেলে দিতে পারে। সুতরাং এর ব্যবহার অত্যন্ত বিবেচনার সাথে করতে হয়। ডাক্তারের পরামর্শ ছাড়া এই ওষুধের প্রয়োগ কোন মতেই করা উচিৎ নয়। 

আর একটা জিনিস লক্ষ্য রাখতে হবে, Oxford বিশেষজ্ঞদের বিবৃতি অনুযায়ী, এই ওষুধ শুধুমাত্র critical রুগীদের জন্যে কার্যকরী। স্বাভাবিক সুস্থ মানুষের জন্যে বা উপসর্গ নেই এমন করোনা সংক্রামিত রুগীর জন্যে নয়। 

Decamethasone কি খুব খরচ এর ওষুধ??

একেবারেই নয়। এই ওষুধ সারা বিশ্বে বহুল পরিমাণে ব্যবহৃত হয় এবং এর খরচ খুবই সামান্য। এক একজন রুগীর ক্ষেত্রে এই ওষুধের প্রয়োগ সীমা প্রায় ১০-১৫ দিন হতে পারে। 

১০ দিন সমপরিমাণে dexamethasone ওষুধের প্রয়োগে প্রত্যেক রুগীর জন্যে মাত্র ₹৪৮০ টাকা থেকে ₹৬০০ টাকা অব্দি খরচ। স্থানভিত্তিতে ও আনুসঙ্গিক উপাদানের প্রয়োজনীয়তার ভিত্তিতে এই খরচ বাড়তে বা কমতে পারে। শুধুমাত্র Dexamethasone একা কার্যকরী হয় না।

এই ওষুধের কি কি side effects রয়েছে??

Dexamethasone এর side effects এর তালিকা বেশ দীর্ঘ। কিন্তু অতি প্রয়োজনীয় মুহূর্তে জীবন দায়ী ওষুধ হিসেবে এর side effect এর কথা ভেবে নিয়ে তবেই প্রয়োগ করা হয়। যদিও জীবনের সামনে সেই সব side effects কে নগন্য বলেই মনে হয়। কিন্তু একজন সুস্থ স্বাভাবিক মানুষ যদি অকারণে এবং না জেনে নিজের শরীরে এর প্রয়োগ শুরু করে দেন তাহলে স্নায়বিক দুর্বলতা থেকে শুরু করে আরো একাধিক খারাপ উপসর্গ দেখা দিতে পারে। 

করোনা সংক্রমণের এই সঙ্কটকালে এই খবর যথেষ্ট স্বস্তির। 

এই ওষুধের প্রয়োগ এবং তার ফলাফল সম্পর্কে সম্পূর্ণ রূপে কোন গবেষণা লব্ধ ফলাফল এখনও পাওয়া যায়নি। করোনা রোগের কোন প্রতিষেধক বা সঠিক চিকিৎসা পদ্ধতির সম্পর্কে সম্যক ধারণার অবর্তমানে এই আবিষ্কার সত্যি একটি চাঞ্চল্যকর খবর। আশা করা যায় খুব শীঘ্রই আমরা এই সঙ্কট কে দমিয়ে আবার স্বাভাবিক জীবনে অভ্যস্ত হয়ে উঠবো।

An era of Digital Thermometer was emerging with its own pace. Suddenly the pandemic changed its course into a 'must have' object in every household, organizations or mass gathering areas. 

The image of thermometer that we usually plot in our mind is a probe like device to measure the temperature of an object. It can be placed under your tongue or can be pushed into a loaf of meat to do the job. Probe style thermometers were a standard version to measure temperatures. In recent years several other methods are emerging daily. Among all of them, infrared thermometers are the most exciting and useful style to be considered now a days.

The IR thermometer can be used anywhere anytime to measure the surface temperature of an object.

What is InfraRED?

Red is the colour, and Latin word 'Infra' means below.

Infrared is just one type of radiation that exists within the range of electromagnetic spectrum. 

The wavelength of infrared rays is longer than the visible spectrum. If we look into the electromagnetic spectrum it would appear just after the red area of the visible lights spectrum and thus, infrared.

IR radiation is quite literally heat. Our eyes cannot detect infrared visibly but we can feel it. The sizzling fried chicken, the hot cup of tea that we experience with our hands is interaction with infrared radiations.

This IR radiation can be measured and representing that measurement electronically is the basic principle of Handheld digital infrared thermometers. 

How Infrared radiation is used to determine temperature?

If there is a difference of temperature between objects, all matter emits energy in the form of Infrared or heat. 

If any object in question (OiQ) is at the same temperature with its surroundings the net radiation energy exchange will be zero. 

Handheld IR thermometers take this advantage of radiation dependence on temperature and produces a value of the targeted object.

How it works?

Infrared light works like the visible light and can be focused, reflected and absorbed.

IR thermometers typically use a lens to focus light from an object on to a detector. 

This sensor or detector is called as Thermopile.

Thermopile absorbs the IR radiation and turns it into heat. The more radiation, the more heat it produces. In a sequence this heat is converted into electrical energy which helps the monitor to measure the electricity. The more radiation means more heat from the thermopile, and more heat means more conversion of energy into millivolts. More millivolts signals the monitor and represented as a temperature reading on the screen.

To be precise, it is a combination of thermocouples arranged in a series or in parallel. When dissimilar metals are exposed to a temperature difference it generates a voltage. This is the basic principal of thermoelectric effect.

To be more specific:

Fast reading- The IR thermometer must be fast with its processing to give the accurate results. Ideally it should be less than 3 seconds.

Accuracy- Measuring the accurate surface temperature mostly depends upon the D/S ratio. But the difference of accuracy should be within the range of ± 0.2-0.4℉ .

Distance to Spot ratio- D/S ratio is the size of the area evaluated by the thermometer related to the distance. The smaller the target, the closure you should be to measure the temperature. 

This ratio have a significant impact on the accuracy and precision of the reading. 

D/S ratio is represented as 30:1 or 5:1 or 8:1 or 12:1 etc. 

For an example, if your target size is 4 inches and your thermometer has a D/S ratio of 5:1. Then the maximum distance that you can reliably stand to maintain the safe distance is 20 inches. Means 1' 6". 

The larger D/S ratio the accurate measurement you can get for an object or target with its significant size.

Why it is important??? The smaller D/S ratio thermometers will get confused to measure the accurate temperature of an object with its surroundings. 

What is the standard D/S ratio? In medical fields a thermometer with a D/S ratio of 12:1 is enough to maintain a safe distance. 

Example, to measure the body surface area of 2 inches you can stand 2 feet away from the target surface. This is the main reason to select a thermometer with great range of D/S ratio. 

Joke: You do not have to buy the 100:1 D/S ratio thermometer for your medical practice. It only works really well if you are near a volcano. 

Types of Digital thermometers

1) Digital forehead thermometers: It is also known as Temporal artery thermometer. It uses the infrared technology to measure the surface temperature of the skin on temporal or forehead areas. Temporal Artery Thermometry was compared with Pulmonary Artery Thermometry (PAT) and it showed 0.1℉ - 0.6℉ accuracy. 

 

2)  Digital Ear Thermometer: It is used to measure the body temperature by placing the concerned head inside the ear. Another version of it is also known as behind the ear thermometer. 

 

 3) Digital sticks: It is the most common device that we encounter day to day in our life. It can measure the body temperature if placed under the arm.

 

 4) Pacifier thermometer: These are mostly used for infants

How to verify Infrared Thermometer Accuracy?

The basic concept is an Infrared thermometer can never be as accurate as the probe type thermometers. It can never show 98.6℉ body temperature as it is not measuring the core temperature. It may show within a range from 97.5℉ to 98.3℉.

Some of the methods mention about boiling water test to check the accuracy of an IR thermometer.

Boiling water is not a valid option as the steam may cause damage to the lens.

Instead of boiling water test it is suggested to check your infrared thermometer with Ice water test.

Procedure

> The thermometer must be in room temperature.

> The sensor lens must be free of dust or any debris. 

> D/S ratio. If the D/S ratio of your thermometer is 12:1, then you can measure the 1 inch diameter area from 12 inches distance. 

> Field view. The IR sensor must be positioned at  90° angle with the target surface to prevent the sensor take the temperature from the surroundings.

> Fill a glass with ice cubes and add enough water just to fill the gaps between the ice. Wait for 2-3 minutes then stir gently for a uniform roll. Point the thermometer at 90° angle at 3" distance and push the button. If the thermometer is accurate it will show 32℉ on screen.
